CleanSlate Southwest Earns CARF® Accreditation For Addiction Treatment Program Excellence

ARIZONA (January 7, 2020) – CleanSlate Outpatient Addiction Medicine, a national medical group providing integrated medication-assisted treatment and behavioral health therapies for the chronic disease of addiction, recently received three-year accreditation by the Commission on Accreditation of Rehabilitation Facilities (CARF) for Office-Based Opioid Treatment: Addiction Pharmacotherapy (Adult Program) certification for its Arizona centers. CARF ® accreditation demonstrates a program’s commitment to the highest quality treatment methods.

To earn accreditation, CleanSlate centers in Arizona underwent a rigorous peer review process including a combination of on-site visits, leadership surveys, interviews with care teams and patients, and operations reviews. CleanSlate demonstrated its commitment to offering programs and services that are measurable, accountable and of the highest quality.
